How care is coordinated with the physician

Care follows the patient’s individualized plan of care and the orders of the patient’s physician or other qualified healthcare provider. The social worker shares what matters with the care team so support reflects the whole person, not only the diagnosis.

How to request an evaluation

Call our office at 281-925-7621 or use our short contact form, and a member of our team will follow up during regular business hours. Referring providers can send a referral directly.

Home health care is prescribed by a qualified healthcare provider and is based on each patient’s clinical needs, eligibility, and individualized plan of care. Eligibility, visit frequency, coverage, and results vary and cannot be guaranteed.

Let us help you determine whether home health care may be right for you or someone you love.
