Patients & families
What to Expect
Open each step to see what happens and to ask the questions families often have.
Home health care is prescribed by a qualified healthcare provider and is based on each patient’s clinical needs, eligibility, and individualized plan of care.

What happens at each step
STEP 01
A provider orders home health
Home health care is prescribed by a qualified healthcare provider, who decides whether it may be appropriate.
What would I want to know?
Can I ask for home health myself?
Home health is ordered by a qualified healthcare provider. You are welcome to call us, and our team can explain the process.
What if the patient is in the hospital?
Hospitals and case managers can refer patients to us. Our Healthcare Professionals page explains how.
STEP 02
You speak with our team
Call our office or use “Speak With Our Team.” A member of our team will follow up during regular business hours.
What would I want to know?
What do I need to share?
Only your name, telephone number, whether you are a patient, family member, or healthcare professional, your city or ZIP code, and how you prefer to be contacted. Please do not send diagnoses, medications, Medicare numbers, or medical records through the website.
Can I just call?
Yes. Our office telephone number is always shown on the website.
STEP 03
We review the referral
Our team can explain the home health process and determine whether we have the information needed to evaluate a referral.
What would I want to know?
What does Medicare look for?
Eligibility is based on each patient’s circumstances, including the need for part-time or intermittent skilled care, homebound status, an order from a qualified healthcare provider, and care provided by a Medicare-certified home health agency.
STEP 04
An evaluation
We arrange an evaluation to understand the patient’s clinical needs, abilities, and goals.
What would I want to know?
Can I say what matters to me?
Yes. Every plan of care is personalized around the patient’s clinical needs, abilities, and goals.
STEP 05
A personalized plan of care
Our clinical team works closely with patients, families, physicians, hospitals, and caregivers to provide coordinated care at home.
What would I want to know?
Who is on the care team?
Depending on the plan of care: skilled nursing, physical, occupational, and speech therapy, medical social work, and home health aides.
How often will clinicians visit?
It depends on each patient’s plan of care, so visit frequency cannot be promised in advance.
STEP 06
Safe, independent living at home
Our goal is to help every patient live as safely and independently as possible, with clear communication along the way.
What would I want to know?
What if needs change?
We provide care that is responsive to each patient’s changing needs, and we coordinate with physicians, patients, families, and caregivers.
Are results guaranteed?
No. Eligibility, coverage, visit frequency, and outcomes cannot be guaranteed.
Home health care is prescribed by a qualified healthcare provider and is based on each patient’s clinical needs, eligibility, and individualized plan of care. Visit frequency and outcomes vary by patient and cannot be guaranteed.
