An order from a qualified healthcare provider
Patient information our team can use to determine whether we have what we need to evaluate the referral
Medicare is our only accepted payer. We do not accept private insurance or other plans
Eligibility is based on each patient’s circumstances, including the need for part-time or intermittent skilled care and homebound status
