Patient Rights and Responsibilities

Effective October 2026

Your rights as a patient

  • To be treated with dignity, courtesy, and respect, and to have your person and property respected.
  • To be free from mistreatment, neglect, abuse, and misappropriation of property.
  • To be told in advance, in a way you can understand, about the care we will provide, who will provide it, and any changes to your plan of care.
  • To take part in planning your care, and to make decisions about it, including refusing care to the extent permitted by law.
  • To have your health information kept private, and to receive our Notice of Privacy Practices.
  • To be told which of our services Medicare covers, and what, if anything, you may be responsible for.
  • To receive information in a language and format you can understand, free of charge.
  • To voice concerns or complaints about your care without fear of discrimination or reprisal, and to have them reviewed promptly.

Your responsibilities

  • Give us accurate and complete information about your health, medications, and eligibility.
  • Follow your plan of care, and tell us if you are unable to or if your condition changes.
  • Tell us in advance if you will not be home for a scheduled visit.
  • Let us know if you do not understand something, so we can explain it.
  • Treat our staff with courtesy and respect, and help provide a safe home environment for visits.
